A 5 mg vial of Cagrilintide dissolved in 2 mL of bacteriostatic water is 2,500 mcg per mL, which is 25 mcg per unit on a U-100 insulin syringe.

Cagrilintide is an investigational once-weekly amylin analog and is not FDA-approved, so there is no label, no manufacturer reconstitution instruction, and no reference concentration to copy. It is also not a GLP-1 agonist, which matters because it comes in 5 mg and 10 mg vials, two of the same sizes as semaglutide, and the two get read off the same tables. A semaglutide table only gives the right units for cagrilintide when the vial mass and water volume happen to match exactly.

How much bacteriostatic water for a Cagrilintide vial?

There is no single correct volume. More water gives a weaker solution, so each unit on the syringe holds less Cagrilintide and a given amount takes more units to measure, which is easier to read on the syringe scale. Less water does the opposite. These tables show the concentration for each common vial size across the usual volumes. They are arithmetic, not a recommendation of how much to take.

How many units of Cagrilintide is a given amount?

Divide the amount in micrograms by the micrograms per unit for your mix. With a solution of 2,500 mcg per mL, one unit on a U-100 syringe holds 25 mcg, so any amount in mcg divided by 25 gives the units. The calculator above does this for you from your own vial and water, and the tables give the per-unit figure for each common mix.

Why does Cagrilintide need its own calculation?

  • 5 mg in 2 mL is 2,500 mcg/mL, so 1 mg is 40 units. A 10 mg vial in the same 2 mL is 5,000 mcg/mL, where the same 1 mg is 20 units. Trial amounts are written in mg, so enter mg and let the tool convert rather than adding zeros by hand.
  • CagriSema, the combination filed with the FDA in December 2025, is cagrilintide and semaglutide co-formulated by the manufacturer at a fixed ratio in a single injection. Two research vials are not that product: each has its own concentration, and a unit count from one never applies to the other. The blend calculator works out both side by side.
  • Cagrilintide carries a fatty-acid chain that slows its clearance, which is why it was developed for once-weekly injection. With one draw a week, the reconstituted stability window can end a vial before the peptide runs out, and the vial longevity calculator turns a vial into weeks.

Cagrilintide is a once-weekly lipidated amylin analog studied both as a monotherapy and as the amylin half of the fixed-dose combination CagriSema. It is investigational, so there is no marketed standalone presentation and no labeled titration schedule.

The fatty-acid acylation is what makes weekly administration possible; it slows clearance of a hormone analog that would otherwise be far shorter-acting.

Novo Nordisk filed an NDA for the CagriSema combination in December 2025. Cagrilintide alone remains unapproved.

Is cagrilintide FDA-approved?

No. Cagrilintide on its own is investigational and has no approved product. The combination CagriSema was submitted to the FDA in December 2025, and the legal status page tracks where it stands. A lyophilized research vial is not an approved product of any kind.

How many units is 1 mg of cagrilintide?

On a 5 mg vial in 2 mL (2,500 mcg/mL), 1 mg is 1,000 mcg, which is 40 units on a U-100 syringe. On a 10 mg vial in 2 mL (5,000 mcg/mL) it is 20 units. 1 mg is used here as a round number for the arithmetic, not as a reference amount.

Can I use a semaglutide reconstitution chart for cagrilintide?
