What the three statuses mean.
A sponsor submitted manufacturing, safety and efficacy data, the FDA reviewed it, and the agency cleared a specific product for a specific use. This is the strongest regulatory position anything on this site holds. It brings a printed label, a manufacturing standard, post-marketing surveillance, and a named party who is accountable for what is in the vial.
It is also narrower than the word suggests. Approval attaches to an indication, a population, a formulation and a route. It does not certify the molecule in general, and it does not transfer to a compounded or research-grade version of the same active ingredient. Using an approved drug outside its labelled indication is off-label prescribing, which is legal for a physician to do and is not the same as that use being supported by the trial data.
The compound is in registered human clinical trials, or is an approved medicine somewhere other than the United States. There is a protocol, an ethics committee, a monitored adverse-event database, and a sponsor with something to lose. That structure is why investigational compounds generate real human data while research-use-only compounds mostly do not.
Being in trials is not a verdict. Most drug candidates that enter clinical development never reach approval, and a few compounds in this table were halted for futility or on safety grounds after reaching late-stage trials. An investigational label tells you a question is being asked properly, not that the answer came back the way anyone hoped.

It is not a purity claim, not a safety statement, not a grade of quality, and it certainly does not mean a regulator checked the vial.
Practically, nobody is guaranteeing that the contents match the label, or the sterility, or the endotoxin load, or the absence of a different peptide entirely. Read safety and quality for what independent testing has found when people went looking, and the legal page on any compound row for the specifics.
If you take one thing from this page, take this: the status column and the note column have to be read together. “FDA-approved” on its own is close to meaningless, because it never means approved-in-general. Oxytocin is approved for obstetric use. Pentosan polysulfate is approved for interstitial cystitis. Afamelanotide is approved to increase pain-free light exposure in a rare porphyria, and explicitly not for tanning. Every one of those is a green badge in the table above, and none of them is an endorsement of the use you probably read about.
FAQ.
Does FDA-approved mean the peptide is approved for what I read about online?
Almost never. Approval is granted for a named product, in a named formulation, for a named indication, in a named population. Semaglutide is approved for type 2 diabetes and chronic weight management, not for every metabolic outcome it gets discussed for. Tesamorelin is approved for HIV-associated lipodystrophy, which is a long way from general body composition. Reading the status column without reading the note column will mislead you.
What does research-use-only actually mean?
It means the compound has no FDA approval for human use, and vendors label it for laboratory research to stay on the right side of that fact. That label is a legal position taken by the seller. It is not a purity claim, not a safety statement, not a quality grade, and not a sign that any regulator inspected the vial, its sterility, or whether the contents match the label.
Is investigational better than research-use-only?
It is a different thing, not automatically a better one. Investigational means the compound is in registered human clinical trials, so there is a sponsor, a protocol, a monitored safety database and a regulator watching. That structure produces real human data over time. It does not mean the compound worked, was found safe, or will ever be approved. Several compounds in this table reached late-stage trials and were then discontinued.
Where does this data come from?
It is the same data published on each compound profile and each legal page on this site, arranged as one table. Status buckets come from the per-compound regulatory record, and the note names the actual approved product and indication where one exists. Nothing in this table is a new claim, and no efficacy judgement is expressed here at all.
Can a compound move between these buckets?
Yes, and several will. Approvals are granted, indications are added, trial programmes are discontinued, and products are withdrawn. This table is a conservative snapshot of the public record, not a live regulatory feed. For anything that matters, check the current FDA label or the trial registry directly.
Does approval in another country count?
Not in this table's status column, and the distinction is worth understanding. Thymosin alpha-1 is an approved medicine in several countries, and Cerebrolysin and Cortexin are registered in others, but none of them are FDA-approved, so they sit in the investigational bucket with the foreign registration named in the note. Approval is a jurisdiction-by-jurisdiction fact.
