Best peptides for gut health.

A small category with an unusual feature: one of its compounds actually completed late-stage human trials, and the result was not what the field hoped.

Gut-directed peptides target a genuinely important problem. Intestinal barrier integrity, the tight junctions between epithelial cells that decide what crosses from the gut into the body, is implicated in celiac disease, inflammatory bowel conditions and a long list of speculative associations beyond them. A compound that could tighten that barrier would be clinically valuable.

Larazotide is the compound that actually tried. It is an orally administered peptide designed to act locally in the intestine rather than being absorbed, regulating tight junction assembly in celiac disease. It produced a positive result in a randomized Phase 2b trial in patients still symptomatic on a gluten-free diet, then advanced to a large Phase 3 program. That Phase 3 trial was halted after an interim analysis indicated it was unlikely to meet its primary endpoint.

That sequence is worth sitting with, because it is what the honest version of drug development looks like. A promising mechanism and a positive mid-stage trial did not survive a properly powered late-stage test. Most compounds on this site have never been exposed to that risk, which is not the same thing as having passed it.

The remaining compounds in the category are earlier. Some have interesting preclinical gut data, and the localized nature of gut delivery is a mechanistically reasonable argument for why oral administration might work where systemic absorption would not. None has completed the kind of trial that would settle the question.

The honest verdict

The most-studied compound here reached Phase 3 and missed its primary endpoint. The rest are preclinical. This category has better evidence quality than most on the site and a less encouraging result because of it.

Does oral BPC-157 work for gut issues?

The rationale is that it may act locally on gut tissue without needing systemic absorption, and there is preclinical gut-protective data. Oral bioavailability and human efficacy are both unestablished, and most of the underlying animal work used injection.

What happened with larazotide?

A randomized Phase 2b trial in celiac patients with persistent symptoms showed benefit at the lowest dose tested. The subsequent Phase 3 trial was stopped at interim analysis for likely futility on its primary endpoint, and the program ended. It is not approved.
