Best peptides for cognition.

Where the outcomes are subjective, the placebo response is enormous, and the compound with the most trial evidence produced the least encouraging result.

Cognitive peptides present a measurement problem that other categories avoid. The outcomes people care about, focus, clarity, motivation and mood, are subjective and unusually responsive to expectation. Placebo effects in cognitive and psychiatric research are among the largest in medicine, which makes personal experience a weaker guide here than anywhere else on this site.

The category also has a distinct geography. Several of its best-known compounds were developed in Russia, are used clinically there, and have a substantial domestic literature while remaining unapproved and largely unstudied in Western trials. This produces an awkward evidence position that resists simple summary: it is wrong to call these compounds untested, and equally wrong to treat clinical use under one regulatory system as equivalent to a Western approval package.

Cerebrolysin is the most informative case on the entire site for what good evidence looks like when it does not favour a compound. It has been through many randomized controlled trials and two Cochrane systematic reviews, far more scrutiny than nearly anything else here receives. The 2023 review of acute ischaemic stroke concluded it probably has little or no beneficial effect on death or clinical outcome, and flagged a possible increase in non-fatal serious adverse events. The 2019 vascular dementia review found possible short-term benefits but did not recommend routine use, citing small trials and risk of bias.

The purely preclinical members of the category have interesting mechanisms and no human data. Blood-brain barrier penetration recurs as an unresolved question, since a compound that cannot reach the brain cannot produce a central effect regardless of what it does in a dish. One prominent compound in this space had a key mechanistic paper retracted, which is worth knowing before reading confident claims about it.

The honest verdict

The compound here with the most and best human evidence is also the one whose systematic reviews concluded it probably does not help. That is what this category looks like when it is examined properly.

Does Cerebrolysin work?

Two Cochrane reviews examined it. The 2023 stroke review concluded it probably has little or no beneficial effect and noted a possible safety signal. The 2019 vascular dementia review found possible short-term benefit but did not recommend routine use.

Are Semax and Selank proven?

They are used clinically in Russia with a domestic literature behind them, and lack independent Western randomized trials. That is a genuine evidence gap rather than a verdict that they do not work.

Why does personal experience count for less here?

Because cognitive and mood outcomes are the most placebo-responsive endpoints measured in medicine. Feeling sharper after taking something does not establish that the something caused it.
