Mitochondrial peptides come from a legitimately interesting discovery: the mitochondrial genome encodes small peptides that act as signalling molecules, not just the proteins involved in energy production. That finding is roughly a decade old and opened a genuine research field around how mitochondria communicate with the rest of the cell.
The clinical translation is at very different stages across the category. One compound has reached real clinical development for a defined rare mitochondrial disease, with the trial infrastructure that implies. Others remain where they started, which is animal work and human biomarker association studies showing that circulating levels of the peptide correlate with exercise and metabolic health.
That distinction between correlation and intervention matters enormously here and is routinely collapsed in marketing. Observing that people with better metabolic health have higher levels of a mitochondrial peptide does not establish that administering the peptide improves metabolic health. The causal arrow could run either way, or both could follow from a third factor such as exercise.
Fatigue and energy are also, like sleep and cognition, subjective endpoints with large placebo responses, which makes the personal-experience evidence that circulates about this category particularly weak. The compounds with the most credible claims here are the ones being developed for specific diagnosable mitochondrial conditions rather than for general energy.
Real and interesting biology, one compound in genuine clinical development for a defined disease, and a marketing layer that consistently converts biomarker correlation into claimed benefit.

Does MOTS-c increase energy?
Human studies have largely measured it as an exercise-responsive biomarker rather than testing it as a treatment. Correlating with metabolic health is not the same as improving it when administered.
What is the best-evidenced compound here?
