How long does Pentosan Polysulfate stay in your system?
Pentosan Polysulfate has a published half-life of 27 hours (300 mg) and 20 hours (450 mg). By the standard pharmacology rule of thumb, a drug is about 97% eliminated after roughly five half-lives, which here works out to between 4.2 and 5.6 days. Both values track total plasma radioactivity after a radiolabelled oral dose rather than intact drug, so neither is a clean elimination half-life.

What is the half-life of Pentosan Polysulfate?
The Elmiron prescribing information reports a half-life of 27 hours at one oral dose level and 20 hours at the other, from a study that followed total plasma radioactivity after a radiolabelled oral dose. Pentosan polysulfate is a semisynthetic, heparin-like sulfated polysaccharide derived from beechwood, approved as Elmiron for the pain of interstitial cystitis.
Read this one carefully. Radioactivity follows the radiolabel wherever it goes, including into breakdown products, so the decline it measures is not the decline of intact drug. Only about 6% of an oral dose is absorbed in the first place. The figure is real and label-sourced, but it is not the same kind of number as the other elimination half-lives in this cluster.
How much Pentosan Polysulfate is left after each half-life?
Each half-life removes half of whatever is left, so the percentages are pure arithmetic and the same for any compound; only the clock changes. For Pentosan Polysulfate, one half-life is 20 to 27 hours. The rows below run that forward for a single amount with nothing added.
| Half-lives elapsed | Time elapsed | Remaining | Cleared |
|---|---|---|---|
| 1 | 20 to 27 hours | 50% | 50% |
| 2 | 40 to 54 hours | 25% | 75% |
| 3 | 2.5 to 3.4 days | 12.5% | 87.5% |
| 4 | 3.3 to 4.5 days | 6.25% | 93.75% |
| 5 | 4.2 to 5.6 days | 3.13% | 96.88% |
| 6 | 5 to 6.8 days | 1.56% | 98.44% |
| 7 | 5.8 to 7.9 days | 0.78% | 99.22% |
Why do the numbers differ between sources?
Within the label there are already two numbers, 27 and 20 hours, for two dose levels. Outside it, figures for pentosan polysulfate may describe other routes or the osteoarthritis research in which the molecule is also studied, and none of those transfer to the oral product for interstitial cystitis.
Because the labelled value tracks radioactivity, any figure presented as the half-life of intact pentosan polysulfate needs its own primary source. We have not found one we are willing to cite, so this page does not offer one.
What does a half-life not tell you?
It does not tell you where the drug goes. Much of what is absorbed distributes to connective tissue and the urinary tract, and the bladder lining is where the drug is thought to act. A plasma radioactivity curve says nothing about how long drug persists at that site.
It also says nothing about long-term risk. The pigmentary maculopathy linked to Elmiron is associated with high cumulative exposure over years, which is a question of total use over time, not of how fast a single dose declines.
Can you run the numbers yourself?
The estimator applies the same decay arithmetic to any half-life, starting amount and time you enter. It opens with Pentosan Polysulfate's published value loaded. Type another figure to see how sensitive the result is to it.

What are the limits of this estimate?
Everything above is a single-compartment approximation: one pool of drug, falling by the same fraction in every interval. Real elimination varies with kidney and liver function, body composition, formulation and route, and a published figure describes only the route and the people it was measured in. Many compounds also show a fast distribution phase before the slower elimination the table describes.
A half-life is not a dosing schedule. Labelled intervals come from clinical trials that measured outcomes and tolerability, and nothing on this page suggests when to take, repeat or stop anything. Whether Pentosan Polysulfate is out of your system for drug-testing purposes is a different question again. Tests often look for metabolites rather than the parent compound, and the answer depends on the assay, its sensitivity and the sample type, none of which a plasma half-life captures.
FAQ.
Why does the Elmiron label give two half-lives?
The label reports 27 hours and 20 hours for two oral dose levels in a radiolabelled study. Both describe the decline of total plasma radioactivity rather than intact drug, which is why neither is a clean elimination half-life.
How much of an oral dose of pentosan polysulfate is absorbed?
Only about 6%, according to the label data summarised in our half-life reference. Most of an oral dose never enters the blood, which is another reason a plasma figure captures only part of the picture.
Does the half-life explain the retinal warnings?
No. The maculopathy associated with Elmiron is linked to high cumulative exposure over long periods. A half-life describes a single dose declining, and it cannot speak to cumulative risk.
