Pentosan polysulfate is the only oral drug FDA-approved specifically for interstitial cystitis and bladder pain syndrome, so people rarely leave it for lack of evidence. They leave because of the eye. In June 2020 the FDA updated the Elmiron label to warn of retinal pigmentary changes, a maculopathy risk that has made both patients and prescribers reconsider long-term use. That is a hard search to satisfy, because no other compound holds that same approval, and pentosan's other proposed uses in osteoarthritis and antiviral contexts are themselves investigational and unproven. Alternatives here are best discussed with the prescribing clinician rather than chosen from a mechanism list.
