PEG-MGF alternatives.

10 compounds compared to PEG-MGF, ranked by published evidence tier and regulatory status, not by what's marketed hardest.

Why people look for an alternative

PEG-MGF stacks two problems, and people usually discover them in order. The first is the base molecule: the MGF E-peptide's proposed effect on muscle stem cells failed to replicate in a controlled cell study that included the positive control much of the earlier literature lacked. The second is the modification: polyethylene glycol conjugation raises tissue-accumulation and anti-PEG antibody questions that have never been examined for this molecule. It is research-use-only, not for human consumption, and WADA prohibits MGF under category S2. So the search is for a growth-factor approach with evidence rather than a longer half-life. Mecasermin, approved native IGF-1, is the only approved molecule nearby.

PEG-MGF is a PEGylated synthetic peptide based on the unique C-terminal E-domain of mechano growth factor (MGF), an alternatively spliced isoform of insulin-like growth factor-1 known as IGF-1Ec (the rodent equivalent is IGF-1Eb).

How this list is built

An “alternative” here means a compound that shares PEG-MGF's mechanism pathway or sits in the same category, not a marketing substitute. Each is ordered by PepCue's evidence tier (a weighted 0–100 score across human data, safety, replication and regulatory clarity), then by whether it's actually approved. This is a research reference, not medical or purchasing advice: an alternative sitting higher on evidence does not mean it's right, safe, or legal for any given use. Reviewed June 1, 2026.
