Why people look for an alternative

MGF is usually bought on the idea that a locally produced IGF-1 splice variant kick-starts muscle satellite cells after loading. The reason people go looking for an alternative is that the idea did not hold up: Fornaro and colleagues found the isolated E-peptide had no apparent effect on myoblast or human muscle stem cell proliferation, while mature IGF-1 did. So the search is often for the same growth-factor effect with evidence attached. The only approved molecule anywhere near this pathway is mecasermin, recombinant native IGF-1 for severe primary IGF-1 deficiency, which is a different molecule for a different problem. Nothing in the MGF family has human outcome data.
