LL-37 alternatives.
4 compounds compared to LL-37, ranked by published evidence tier and regulatory status, not by what's marketed hardest.
LL-37 is not a designer compound. It is the only human cathelicidin-derived antimicrobial peptide, produced by neutrophils, keratinocytes, and epithelial cells as part of innate immunity, which is why it draws interest for wound and infection contexts. The reason people search for an alternative is the gap between that biology and any usable product: it is not FDA-approved, clinical work has been early-phase and indication-specific, notably topical chronic-wound trials, and no LL-37 product holds general marketing approval. Nothing else in the host-defense peptide class holds one either. The compounds listed below overlap with LL-37 on repair and immune signaling as categories, not on demonstrated clinical benefit.
LL-37 is the only human cathelicidin-derived antimicrobial peptide, a 37-residue cationic, amphipathic alpha-helical peptide (named for its two leading leucine residues) cleaved from the C-terminus of the precursor protein hCAP18 (gene CAMP).

An “alternative” here means a compound that shares LL-37's mechanism pathway or sits in the same category, not a marketing substitute. Each is ordered by PepCue's evidence tier (a weighted 0–100 score across human data, safety, replication and regulatory clarity), then by whether it's actually approved. This is a research reference, not medical or purchasing advice: an alternative sitting higher on evidence does not mean it's right, safe, or legal for any given use. Reviewed June 1, 2026.
